We also offer a complimentary 15 minute consultation with the doctor.

In order to keep our services affordable, we have developed the following financial policy.

• For our patients without chiropractic insurance coverage, payment is due at the time services are rendered. Ask about our special discounts, you may qualify!
• For our patients with chiropractic insurance coverage, your co-payment will be collected at the time of services. In addition, you will also be responsible for any amount that your insurance company does not pay. If your deductable has not been met, full payment for services rendered will be collected at the time of service until your deductible is met.

We are an out-of-network provider for:

• Aetna-benefits are paid after an out-of-network deductable is met.
• Aetna HMO-no benefits are paid.*
• Community Care HMO-no benefits are paid.*
• Perferred Community Choice-no benefits are paid.*

MEDICARE PATIENTS

Medicare pays only for the chiropractic adjustment. There are 3 payment options with Medicare. Please refer to Advance Beneficary Notice of Noncoverage by contacting the number on the back of your card.

PERSONAL INJURY

If you were involved in an automobile collisions in which you were not at fault, we will consider taking your case on agreement. We will withhold billing for services until your case is settled or until responsible efforts to settle have been exhausted. In the meantime, the responsible party will be billed. If you have either coverage for medical payments on your auto insurance policy or health insurance, these carriers can be billed as well. You must provide a claim number or health insurance information within 10 days after your first treatment.

WORK-RELATED ACCIDENT

If you were in a work-related accident, your employer's insurance is responsible for your services. You will not be billed for services as long as your case is valid under Oklahoma workers' compensation law.
